At Gateway Dental Plans, LLC we care about your dental health.  We want you to have the best experience possible when you visit your Gateway provider.  Should you have any questions or concerns about your Gateway experience, please call us.  One of you customer relations specialists will work with you one on one to answer any questions you may have, and take a personal interest in resolving any concerns.

  • The plan provides discounts at certain dental providers for dental services.
  • The plan does not make payments directly to the providers of dental services.
  • Plan members are obligated to pay for all dental care services, but will receive a discount from those dental care providers who have contracted with the discount dental plan organization.
  • When making an appointment with a GDP dentist or specialist, please inform them you are a Gateway Dental Plans, LLC member.
  • Gateway Dental Plans, LLC has made every effort to be accurate when listing the directory of dentists and dental specialists.  Due to the dynamic nature of the dental provider network, we strongly suggest you call and verify that the dentist or specialist is a participating provider when scheduling your appointment.  Gateway Dental Plans, LLC does not guarantee care in all areas.  In certain service areas, some types of specialty care may not be available.
  • If a member cancels his or her membership within the first 30 days after the effective date of enrollment in the plan, the member will receive a full refund (minus a thirty dollar processing fee) upon receipt by Gateway Dental Plans, LLC of the member’s written notification of cancellation and the return of the Gateway Dental Plans membership card.  All refund cancellation notices must be sent in writing and received within 30 days of initial purchase via mail to:  Gateway Dental Plans Refunds Department, 4337 G Butler Hill Rd, St Louis, MO 63128.  After 30 days of initial purchase, the Gateway Dental Plans refund policy is as follows:  A member is eligible to receive a pro-rated refund only if:  (all pro-rata cancellation notices must be sent in writing and received at the above address)  Gateway Dental Plans cancels a membership for any reason other than nonpayment of membership fees by the member, or discounted savings are not realized on care delivered by a Gateway provider, or the fee schedule or discount schedule of the plan you selected is not honored by at least one participating general dentist in your vicinity (25 mile radius urban or 50 mile radius rural).  All refunds will be issued within 30 days of receipt of the notice of cancellation.
